Job Description

As a case manager at Uzima, you will:
• Complete intake interviews to determine client needs for services.
• Develop, monitor, and assess client treatment plans and progress by locating resources, establishing client routines, and coordinating services including transportation and housing.
• Advocate and provide personal support in regard to client needs and related services including Idaho Health and Welfare for SNAP, Medicaid, childcare, healthcare, education, and TANF.
• Coordinate additional client care with other Uzima programs as well as with our partner organizations in the community.
Qualifications:
• Must have a bachelor's degree in the social sciences, education, communication, or another humanities field.
• Must be willing to serve diverse communities in a variety of settings.
• Must have a current driver's license, reliable transportation, and proof of insurance.
• Must pass a criminal background check from Idaho Health and Welfare.
• Must possess excellent written and verbal communication skills.
• Must have basic computer skills and be able to utilize Electronic Healthcare Records software.
